Academic Eligibility Requirements
Insurance (Pages 11-16)
- VBCPS is a supplemental insurance to your current provider
- Notify the coach immediately if an athlete is being seen by a doctor or emergency room due to an accident during practice or game
- Claim forms may be issued by the SAC/Trainer.
- Treatment for any injury must be received, and the claim form filed within 90 days of the date of the injury.
- Plan provides 70% of Usual & Customary coverage
- The deductible is $100 per injury.

Parent/Spectator
The following are required of all parent/spectators:
- Realize you represent LKMS, so encourage through positive behavior
- Being a good sport is more important than victory
- Recognize and show appreciation for an outstanding play by either team.
- Be a “team fan” as well as a “my kid” fan.
- Remember that a ticket to a school athletic event is a privilege to observe the contest.
- Treat visiting teams and officials as guests
- No coaching from the sideline. Let the coach “coach”.
- Respect the judgment and integrity of officials, realizing that their decisions are based upon game conditions.
- Bottom line - be a positive role model
Athlete/Coach/Parent Communication
- Appropriate concerns to discuss with the coach include issues of mental and physical well being, strategies for improving individual performance, and issues of behavior.
- Issues not appropriate for a parent to discuss with the coaches include playing time, starting positions, team strategy, play calling, and other athletes.
- Listen carefully to their coaching philosophy today and how they will play team members.
- It is never ok to approach a coach after practice or competition to discuss any of the above. Please refrain and contact according their policies.

Important Dates
General Information
- Any student receiving OSS will automatically be removed from the team.
- After the 1st ISS, the student will not start the next competition, nor will they practice/play the day of suspension.
- After the 2nd ISS, the team member will be removed
- Games begin at 5:30 PM. Please know what time to pick up your child after the games.
- We will provide transportation to away games.
- We ask that you allow your child to ride the bus back to LKMS from away sites.
- Only a parent/guardian may take a child from an away site.
- Cancellations of games and practices will be made after 2 PM. Please refrain from calling after the first rain drop/snow flake.
- There are few dates for make ups, so if at all possible we will play.
- Be sure you sign up for Remind 101 or whatever else the coach is using for communication
- Uniforms are the responsibility of team members as soon as they take possession. Please help ensure we receive them back in the same condition they were issued.
- Transfer rule: 6 or 8 games/matches
- Students are eligible for 6 semesters, which begins when they enter 6th grade for the first time.
- Cell phones must be put in lockers as soon as students enter school. Don’t let this be an ISS.
- $250 fine for giving false information regarding where you live.
- Be positive when using social media (page 8 & 9).

GoFan
GoFan is the easiest and best way to buy tickets for middle and high school events.
How Do Spectators Buy Tickets?
- Visit GoFan.co, search for Larkspur Middle school by following the prompts on the GoFan website.
- Find the event and click on “Get Tickets.”
- Select the number of adults and/or student tickets needed and click “Get Tickets.”
- Select your payment option and then sign in to account to finalize payment.
How Do Spectators View Their Purchased Tickets?
- There are 3 ways spectators can view their purchased tickets.
- Select “View Tickets” in their emailed receipt
- Sign-in to their Go Fan account on a computer
- Open in the GoFan app installed on their personal device
How Do Spectators Redeem Tickets at the Gate?
The ticket holder will show the ticket taker a digital ticket on their phone. Staff working the ticket gate will direct the fan to click the green “Validate” button. The ticket holder must show the big green checkmark on their phone’s screen to show that the ticket is valid.
Can Spectators Buy Tickets at the Gate?
- Yes, tickets will be available for purchase at the gate with a card reader only - there will be no cash sales.
- Tickets can also be purchased by fans using a smartphone to scan a QR at each school. The QR code will direct fans directly to the school’s GoFan website to purchase tickets.
Concussion (Pages 22-24)
General Information
- If your child is suspected of having a concussion during practice or a game and you are not present the trainer or coach will call you. It is mandatory that you come pick your child up, they will not be put on the bus.
- Please be available by phone while your child is practicing or competing.
- You will receive home instructions from person signing your child over to you.
Facts
- Your child will begin the Graduated Return to Play progression only after they are symptom free for 24 hours.
- There are 5 stages and each stage last a minimum of 24 hours.
- If at any time during the stage process they experience any symptoms, the process stops for 24 hours and they start with the last stage where they had no symptoms.
Procedures
If symptoms resolve within 15 mins
- If the original symptoms resolve within 15 minutes of the incident and the student has experienced no other symptoms overnight the Certified Athletic Trainer (ATC) can clear the student to begin the gradual return to play progression the day after the incident without seeing a physician.
- The gradual return to play progression is a minimum of 5 days of gradual progression before full return to play will be considered
If symptoms last longer than 15 mins and/or return overnight
- The student must see a physician
- There is a specific form that must be completed by the physician in order for the student to be considered “cleared to start the 5 day progression”
- This form can be obtained from the school ATC
Sudden Cardiac Arrest
- Is when the heart stops beating suddenly & unexpectedly.
- It is the #1 cause of death for student athletes
- Majority of the conditions are inherited, be aware of your family history.
- If a students exhibits signs/symptoms they will be removed from practice/play and may not return until they see a doctor and return with a note releasing them.
- Please read over and become familiar with the signs/symptoms as well as go over with your child.
Remember
- These guidelines and activity restrictions are for any and all physical activity to include but not be limited to Physical Education Classes, Rec. games and practices, club sports - games and practices.
- Rules are in place to protect the student athlete
- As a parent help the student/athlete take time to get better
- Your support as parents is vital to the success of the program and safety of the students

- Lauren is at Larkspur every day for practice and games
- Inhalers/Epi Pens must be kept in the first aid kit, along with the self carry/self medicate form.
- If your child is seen by a doctor for an injury or illness they must have a note to return to play.

Team Members
- Must communicate problems and concerns with coaches and Lauren
- They must follow her directions for injury.
- She will communicate with you if something happens at practice and/or a game.
